AKISAN USA National Convention Waiver

The 2021 AKISAN USA national convention will be held in Atlanta, Georgia at the Atlanta Marriot Marquis Hotel, 265 Peachtree Center Avenue, NE, Atlanta, GA 30303

Owing to COVID-19, we are actually kicking off this year’s convention on Friday, August 6, 2021. That notwithstanding, we are encouraging everyone to arrive on Thursday, August 5, 2021, to take in the town, visit friends and family, shop and do whatever you need to do, so you are fresh and ready for a fun-filled weekend.

We want all the 2021 national convention attendees to be safe. Though we will be observing and enforcing all COVID-19 protocols as specified by the State of Georgia, we will all be required to read and acknowledge our COVID-19 waiver as part of the requirements for attending the convetion.

The convention is coming up the week of Aug 5th – 8th, 2021. We are trying to get a pulse from the community on which convention format would be preferred: A Hybrid (in-person and virtual) or Virtual convention format.

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing, wearing of face masks, using hand sanitizers, and all other CDC COVID-19 protocols.

I further acknowledge that AKISAN USA has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that AKISAN USA will not be held responsible for being exposed to and/or being infected by COVID-19 due to my actions, omissions or negligence and others.

I voluntarily seek to participate in the activities run by AKISAN USA and acknowledge that I must comply with all set procedures to reduce the spread while participating.

I attest that:

Neither I nor my family members are experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste/smell.
We have not traveled to a highly impacted area within the United States of America in the last 14 days.
We do not believe that we have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
We have not been diagnosed with Coronavirus/COVID-19 or if diagnosed previously with Coronavirus/COVID-19 have been cleared as non-contagious by state or local public health authorities.
I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold AKISAN USA harmless from and waive on behalf of myself, my family, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation. I understand that this release discharges AKISAN USA from any liability or claim that I, my family, or any personal representatives may have against the organization.

This liability waiver and release extends to the AKISAN USA together with all officers and volunteers.
